Mechanisms and rationale for uricase use in patients with gout

Xanthine oxidase inhibitors like allopurinol and febuxostat have long been the go-to therapy for hyperuricemia in gout patients. However, challenges arise as not all can tolerate or use them due to comorbidities and dosage limitations. In response, uricase enzymes, which convert urate to more soluble allantoin, offer a potential solution. While costs and immunogenicity have hindered their use, uricases show promise in swiftly reducing hyperuricemia, resolving tophi, alleviating chronic joint pain, and enhancing the quality of life. This review explores available uricases, their debulking effects, and their potential in gout management.
