MELD 3.0 Enhances Liver Transplant Equity

The MELD score, a cornerstone in liver transplant allocation, has undergone significant evolution to enhance fairness and clinical accuracy. Building on the MELD-Na framework, the latest MELD 3.0 iteration integrates patient sex and serum albumin as variables and adjusts weights for sodium, bilirubin, INR, and creatinine levels. These refinements aim to lower waitlist mortality and improve access for female candidates, addressing previous gender-based disparities. Despite its advancements, global organ allocation systems vary, reflecting diverse healthcare strategies. This review traces the MELD score's development, evaluates MELD 3.0's potential, and explores future directions in liver transplantation allocation to balance fairness and effectiveness.
