A study of 148 patients who underwent Roux-en-Y gastric bypass surgery (RYGB) aimed to understand metabolic changes post-surgery. It revealed that significant alterations in metabolite levels occurred in the first 2 months after RYGB, but after that, they began to revert to pre-surgical levels. Patients achieved a metabolome close to their pre-surgery state when they reached their lowest weight and glucose levels. Long-chain and unsaturated lipids saw more significant changes than shorter, saturated ones, indicating reversible lipid remodelling. This suggests that metabolic remission happens before substantial weight regain, raising questions about its predictive value for future weight and glycemic changes.

Most recommendations remain conditional because evidence certainty is low or very low in several clinical areas.
