In 337 cognitively unimpaired adults followed over ~4 years, increased midlife physical activity and new adherence to WHO guidelines were linked to lower amyloid-PET burden and greater medial temporal lobe thickness. Sedentary behavior was associated with cortical thinning. These findings support promoting physical activity in midlife as a potential strategy to reduce Alzheimer’s disease–related brain changes before cognitive symptoms arise.
