
In a study of 74 ST-segment elevation myocardial infarction (STEMI) patients with large thrombus burden (LTB), researchers explored the safety of deferring stent optimization during percutaneous coronary intervention (pPCI). The study, analyzing outcomes up to 2 years, found that omitting post-dilatation and stent oversizing appeared safe, reducing the risk of distal thrombus embolization (DTE). No major complications were observed during pPCI, and deferred stent optimization led to improved stent expansion and minimal lumen area. Only 1.4% of patients required target lesion revascularization, suggesting that this approach could be a safe strategy for STEMI patients with LTB, potentially improving clinical outcomes.
