Navigating the Cost-Efficacy Spectrum of Extracorporeal Support for Acute Respiratory Distress Syndrome

A study investigated the effects of extracorporeal membrane oxygenation (ECMO) and extracorporeal carbon dioxide removal (ECCO2R) on gas exchange, respiratory mechanics, and hemodynamics in an experimental lung injury model. The results showed that ECMO resulted in better gas exchange and improved hemodynamics, while ECCO2R was associated with worse hemodynamics and higher ventilation requirements for adequate carbon dioxide removal. Extracorporeal support exists on a continuum, and different degrees of support will be needed for different patients throughout their illness. Therefore, it is important to balance the benefits of extracorporeal support against the potential risks, including the increased bleeding risk associated with low flow rates.
