Knee osteoarthritis is a prevalent cause of pain and disability, yet effective treatments are scarce. This triple-blind, randomized clinical trial evaluated the safety and efficacy of placental mesenchymal stromal cell-derived extracellular vesicles in patients with bilateral knee osteoarthritis (grades 2 and 3). The study included 62 knees from 31 patients, who received either 5 cc of extracellular vesicles or normal saline as a control. Symptoms were assessed using VAS, WOMAC questionnaire, and Lequesne index at baseline, 2 months, and 6 months post-injection. MRI scans evaluated cartilage volume and meniscal integrity. The analysis showed no significant differences in clinical outcomes or MRI findings between groups at any time point. Although the treatment was safe with no severe adverse reactions, it offered no additional benefits compared to placebo.
