Non-surgical consecutive exotropia (NCX) is the spontaneous shift from inward to outward eye deviation, without surgery. This study examined NCX cases and their response to conservative management. The study included 49 children with a mean age of 3.5 years at the onset of esotropia (inward deviation) and 8.4 years at exotropia (outward deviation). Notably, high hyperopia was only present in 35.7% of cases. While 60% had accommodative esotropia, all but one showed exotropia at a distance. The hyperopic prescription was reduced in response, but surgery was needed in 43% eventually. The findings challenge hyperopia's primary role, suggesting further investigation into non-refractive factors.
