SGLT2 inhibitors (SGLT2i) significantly reduce major adverse cardiovascular events (MACE) and heart failure (HF) risk in patients with obesity. This study used nationwide Korean data (2014–2022) to assess the effect of baseline BMI on these outcomes. In 231,332 matched T2D patients, SGLT2i use reduced MACE risk (HR: 0.90) and HF hospitalizations (HR: 0.53). BMI modifies the risk reduction, especially in obese and overweight groups, with no effect in normal weight.
