
The literature increasingly highlights the "obesity paradox," suggesting obesity's potential protective effect on mortality. To explore this in non-dialysis chronic kidney disease (ND-CKD) patients, the study analyzed the Salford Kidney Study database (2002–2016). Patients were categorized by BMI and cardiometabolic comorbidities. Obesity positively impacted outcomes in ND-CKD without diabetes, persisting after adjustments (HR 0.74; p=<0.001 and HR 0.48; p=0.008 for CKD alone and CKD+HF groups). However, this protective effect vanished in ND-CKD with diabetes, emphasizing diabetes as a strong predictor of adverse outcomes, irrespective of BMI.
