Obesity is increasingly prevalent among heart failure (HF) patients and exacerbates disease presentation and outcomes, especially in HF with preserved ejection fraction (HFpEF). This European Society of Cardiology scientific statement reviews the epidemiology, mechanisms, diagnostic challenges, and treatment strategies of obesity in HF. It also highlights emerging therapies, such as weight loss interventions, and urges clinicians to adopt tailored approaches for managing HF in obese patients.
