
A post hoc analysis of the NOBILITY trial explored the impact of adding obinutuzumab to standard lupus nephritis (LN) therapy. Results indicate a 60% reduced risk of unfavourable kidney outcomes, 57% lower LN flare risk, and 80% to 91% fewer declines in the estimated glomerular filtration rate. Obinutuzumab users experienced a significantly slower eGFR decline, boasting a 4.1 ml/min/1.73 m²/year slope advantage. Moreover, 38% achieved complete renal response (CRR) at week 76 with ≤7.5 mg/day prednisone, compared to 16% on placebo. The findings suggest that obinutuzumab enhances LN management and kidney function preservation with reduced glucocorticoid dependency.
