Weight gain during pregnancy should be closely monitored for extremely obese women, particularly in late preterm gestation. This retrospective study examined 94 extremely obese pregnant women (BMI ≥ 40 kg/m²) delivering at Seoul National University Bundang Hospital. Results revealed higher chronic hypertension and adverse neonatal respiratory outcomes in women with a pre-pregnancy BMI < 35.0 kg/m², highlighting the impact of pre-pregnancy weight and gestational weight gain.
