Over the last 5 years, advancements in the design of antibody-drug conjugates (ADCs) have revolutionized the treatment of advanced-stage solid tumors. ADCs aim to deliver cytotoxic drugs directly to tumor-specific antigens, reducing toxicity compared to traditional chemotherapy. However, ADCs still face challenges such as off-target toxicities resembling the drug payload and on-target toxicities. To enhance safety, ongoing efforts include optimizing dose and treatment schedules through clinical trials, modifying ADC components, identifying biomarkers for toxicities, and developing innovative diagnostic tools. This review explores the factors contributing to ADC toxicities in solid tumor patients and highlights strategies expected to improve tolerability and treatment outcomes in the future.
