PARP Inhibitor Combinations in First Line mCRPC: A Systematic Review and Meta‐Analysis

A meta-analysis comparing the outcomes of metastatic castrate-resistant prostate cancer (mCRPC) patients receiving combination therapy with poly(ADP-ribose) polymerase inhibitors (PARPi) and androgen receptor axis-targeted agents (ARATs) versus placebo/ARATs has revealed promising results. The combination therapy demonstrated a 35% improvement in radiographic progression-free survival (rPFS) in the overall cohort, with greater improvements observed in BRCA1/2-mutated and HRRm subgroups. However, overall survival (OS) benefits were inconsistent across all subgroups. The combination therapy was associated with a higher risk of grade≥3 treatment-emergent adverse events (TEAEs), notably anemia.
