Perioperative Pembrolizumab for Early-Stage Non–Small-Cell Lung Cancer

A phase 3 trial investigated the effectiveness of perioperative pembrolizumab, an immune checkpoint inhibitor, in patients with early-stage non-small-cell lung cancer (NSCLC). The study included neoadjuvant pembrolizumab or placebo given with chemotherapy before surgery, followed by adjuvant pembrolizumab or placebo after surgery. The results showed that the pembrolizumab group had higher event-free survival rates at 24 months compared to the placebo group. Major pathological response and pathological complete response were also significantly higher in the pembrolizumab group. Although overall survival did not differ significantly, the study suggests that combining neoadjuvant and adjuvant immune checkpoint inhibition may provide greater benefits for patients with early-stage NSCLC compared to chemotherapy alone.
