
The American College of Physicians (ACP) has updated its guidelines on pharmacologic treatment for osteoporosis in adults. Bisphosphonates are recommended as the initial treatment for postmenopausal women with primary osteoporosis to reduce fracture risk, based on strong evidence. For men with primary osteoporosis, bisphosphonates are suggested as an initial treatment with low-certainty evidence. In cases where bisphosphonates are contraindicated or cause adverse effects, the RANK ligand inhibitor denosumab can be used as a second-line treatment for both women and men. Additionally, the sclerostin inhibitor romosozumab or recombinant PTH teriparatide can be considered for high-risk fracture cases in women with primary osteoporosis. Individualized approaches are recommended for females over 65 with low bone mass.
