
Acute heart failure is a leading cause of urgent hospitalizations, resulting in increased rates of death and rehospitalization. To address this, effective management of patients before and after discharge is crucial. This includes early detection and treatment of residual or recurrent congestion, initiation of guideline-directed medical treatment before discharge and adjustment to target doses in the early post-discharge phase, and implementation therapy during the hospitalization period. The aim of the Heart Failure Association of the European Society of Cardiology is to provide a summary of recent findings that have implications for the clinical management of patients with acute heart failure in the pre-discharge and early post-discharge phases.
