This study evaluated the prognostic value of tumor regression scores (TRS) and metabolic response in esophageal adenocarcinoma (EAC) patients who underwent neoadjuvant chemoradiation. Among 277 patients, those with a complete response (TRS 0) had longer overall survival (OS). Signet ring cell features and higher SUV uptake at regional lymph nodes were associated with residual tumors and decreased OS. Pathologic complete response was linked to higher OS, highlighting the need for improved treatments for patients with poor metabolic response.
