
A study aimed to evaluate the predictive accuracy of cervical length (CL) measured by transvaginal ultrasound (TVUS) and magnetic resonance imaging (MRI) for preterm birth (PTB) in twin pregnancies complicated by twin-twin transfusion syndrome (TTTS) undergoing selective fetoscopic laser photocoagulation (SFLP). The frequency of short CL ≤2.5 cm was higher with MRI than with TVUS, and while the two imaging modalities correlated overall, the correlation was poor at the shortest cervical lengths. TVUS outperformed MRI in predicting PTB, with pre-op TVUS CL < 2.0 cm being the best predictor of PTB < 32 weeks. The study concluded that although MRI CL measurement correlates well with TVUS overall, TVUS remains the optimal screening modality for CL in pregnancies at high risk of PTB.
