A systematic review was conducted to identify predictive factors for Obstetric Anal Sphincter Injury (OASI) during first vaginal delivery, diagnosed by ultrasound. The review included 21 studies and found that increasing gestational age at delivery, shorter antepartum perineal body length, labor augmentation, instrumental delivery (particularly forceps extraction), shoulder dystocia, episiotomy use, and shorter episiotomy length were associated with US-OASI.
