This randomized clinical trial compared the efficacy of platelet-rich fibrin (PRF) and concentrated growth factor (CGF) in treating mature necrotic permanent teeth with periapical radiolucency. Eighteen patients were treated and assessed using radiographs, CBCT, electric pulp testers, and cold tests over a 12-month follow-up period. Both PRF and CGF groups showed significant periradicular healing, with no substantial differences in lesion size, bone density, or pulp sensibility between groups at 6 and 12 months. The results suggest that both PRF and CGF are effective in preserving mature necrotic teeth through revascularization.
