Primary vs. Interval Surgery for Stage IV Ovarian Cancer

Primary cytoreductive surgery significantly improves survival outcomes over interval surgery in stage IV ovarian cancer. This study found a 5.0-month increase in progression-free survival and a 3.9-month increase in overall survival with primary surgery. Analysis of 2772 patients revealed median survival times of 63.1 months for primary vs. 55.6 months for interval surgery. Benefits were consistent across subgroups, including those with extraperitoneal metastasis like supradiaphragmatic or extra-abdominal lymph nodes.
