This study investigates differences between youth diagnosed with juvenile spondyloarthritis (SpA) who did and did not fulfill the Paediatric Rheumatology International Trials Organisation (PRINTO) provisional criteria. A retrospective analysis of 158 patients meeting axial juvenile SpA (axJSpA) classification criteria revealed that 68% met the PRINTO criteria. The primary differences between those who met and did not meet the criteria were related to peripheral disease manifestations, such as arthritis and enthesitis, as well as HLA-B27 positivity. These findings suggest that modifications to the PRINTO criteria could help better capture youth with primarily axial disease.
