This review highlights the significant alterations in pulmonary physiology during underwater exposure, where immersion and increased ambient pressure impact the cardiovascular and respiratory systems. Factors like thoracic blood pooling, changes in breathing gas pressures and volumes, and variations in ambient pressure stress the heart and lungs. Adequate cardiovascular and respiratory function is essential for safe diving, whether freediving or using underwater breathing apparatus. Unfortunately, few physicians are trained to understand diving physiology and manage related injuries. The review provides an overview of the challenges to the respiratory system during diving, outlines health risks in the underwater environment, and examines dive-related injuries, focusing on pulmonary physiology and pathophysiology.
