The study analyzed data from 1,625 hypertensive patients aged 40–75 years from the Risk Evaluation of Cancers in Chinese Diabetic Individuals (REACTION) study to evaluate the impact of renin-angiotensin-aldosterone system (RAAS) inhibitors on bone health. The cross-sectional analysis showed no significant association between the use of angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) and osteoporosis or fracture risk. Over a mean follow-up of 3.4 years, 478 new osteoporosis cases and 76 fractures were identified, with hazard ratios (HRs) for osteoporosis of 1.005 and 1.077 for ACEI and ARB users, respectively (p > 0.05). Additionally, the duration of RAAS inhibitor use did not correlate with osteoporosis risk or improvement.
