Head and neck squamous cell carcinoma (HNSCC) patients with perineural invasion (PNI) face a poor prognosis. A retrospective study analyzed HNSCC patients with high- or low-risk PNI who received curative-intent treatment between 2010 and 2021. Among the 44 patients, recurrence rates were 31% for high-risk PNI and 17% for low-risk PNI. High local control rates were achieved with adjuvant or primary RT in high-risk PNI patients, regardless of the treatment group. Few failures were observed in electively treated regions.
