In 412 men with high-risk prostate cancer, HDR+ showed lower biochemical recurrence rates than RARP (p ≤ 0.01), but no differences in CRPC, cancer-specific survival, or overall survival. HRQOL was similar, but RARP had worse urinary incontinence, while HDR+ had more urinary irritation/obstruction at 12 months. Both treatments are effective, with distinct urinary side effects.
