Prostate cancer, a significant public health problem, has caused more than 375,000 deaths worldwide in 2020. One of the main tools available to lessen this burden is prostate-specific antigen (PSA) screening. Large, randomized trials have shown that PSA screening significantly reduces the risk of death from prostate cancer. However, PSA screening also incurs substantial harm, most importantly overdiagnosis — the diagnosis of a disease that would not have been diagnosed without screening — and the complications of associated treatment.
