
This study, conducted as part of the European Randomized Study of Screening for Prostate Cancer (ERSPC) trial, aimed to analyze the impact of the first prostate-specific antigen (PSA) level on prostate cancer (PCa) detection and PCa-specific mortality (PCSM) in a population-based screening setting with repeat screenings every 2–4 years. The study included men aged 55–69 years who attended at least one screening visit during a 16-year follow-up. The findings suggest that baseline PSA levels can guide decisions on the repeat screening interval, with a baseline PSA of <1.0 ng/ml in men aged 55–69 years being a strong indicator to delay or stop further screening.
