Remote Screening for Hyposmia to Identify Alpha-Synuclein Aggregation

This study evaluated a remote, staged screening paradigm using smell identification tests and dopamine transporter imaging (DAT-SPECT) to identify individuals with potential alpha-synuclein aggregation. Among 31,293 participants who completed the smell test (UPSIT), 27% had scores below the 15th percentile. Of those undergoing DAT-SPECT, 69% showed reduced dopamine binding, and 55% of these had positive cerebrospinal fluid alpha-synuclein seed amplification assay (aSynSAA). This proportion increased to 70% for participants with more severe hyposmia (UPSIT <10th percentile). These findings support the feasibility of remote screening to identify individuals for further biomarker evaluation and enrollment in disease-modifying clinical trials.
