Retained Metabolic Flexibility of the Failing Human Heart

This study explored metabolic flexibility and oxygen delivery in patients with nonischemic heart failure. The study compared the effects of insulin+glucose infusion (I+G) and Intralipid infusion on cardiac function and energetics. They found that the heart demonstrated metabolic flexibility, with different substrates being utilized depending on the infusion. During I+G, glucose was the primary energy source, while during Intralipid infusion, fatty acids were utilized more. Interestingly, Intralipid infusion resulted in improved myocardial energetics and cardiac function compared to I+G. The study concluded that promoting fatty acid oxidation may be a potential therapeutic strategy for heart failure, challenging the existing approaches that aim to increase glucose oxidation.
