Retinal vascular structure independently predicts the initial treatment response in neovascular age‐related macular degeneration

This study aimed to determine if non-invasive measurements of retinal vascular structure could predict the success of initial intravitreal treatment in patients with neovascular age-related macular degeneration (nAMD). Fifty-eight treatment-naïve nAMD patients underwent retinal vascular structure assessments prior to receiving three monthly injections of aflibercept. Patients were categorized as full treatment responders (FTR) or non/partial treatment responders (N/PR). Results showed that 44.4% of the eyes were FTR. Older age and lower retinal arteriolar fractal dimension (Fd) and venular length-diameter ratio (LDR) were associated with FTR. Logistic regression models indicated that a higher retinal venular LDR was independently associated with a lower chance of FTR. These findings suggest that retinal venular LDR could predict the initial treatment response in nAMD, potentially guiding treatment decisions.
