
Intestinal lymphatic vessels play a critical role in fluid absorption and transport of immune cells, specifically in the small intestine, where lymphatic fluid is rich in lipids and gut-specific immune cells. This review discusses the mechanisms of lipid absorption from epithelial cells to lymphatics and the unique gut microenvironment's influence on these processes. It also examines the role of intestinal lymphatics in immune cell trafficking and inter-organ communication. Furthermore, the review highlights how understanding fat absorption can enhance lipid-soluble drug distribution, including the potential for chylomicron-associated drugs to bypass first-pass liver metabolism for systemic delivery. This comprehensive overview aims to elucidate further intestinal lymphatic–systemic interactions, guiding future research directions.
