Revolutionizing Mild Hidradenitis Suppurativa Trials: Monthly IHS4 Scores for Precise Treatment Evaluation

Hidradenitis suppurativa (HS) is a chronic, recurring inflammatory skin condition marked by painful nodules, abscesses, and tunnels in skin folds. While 66% of patients have mild disease, clinical trials often focus on moderate to severe cases, limiting treatment options for milder cases. Studying treatment efficacy in mild HS faces challenges, such as the intermittent nature of lesions. The study proposes using the continuous international hidradenitis suppurativa severity score (IHS4) monthly, with cumulative scores (IHS4-C) as the primary outcome. This novel approach aims to enhance treatment assessment in mild HS cases and warrants testing in future studies.
