
A study comparing retrograde intrarenal surgery (RIRS) and mini-PCNL for managing 10–20 mm renal stones revealed similar outcomes. Among 176 patients stratified by the SMASH score, RIRS was used for scores <15 (Group A, 90 patients), and mini-PCNL for scores ≥15 (Group B, 86 patients). Both methods showed comparable stone-free rates (87.8% vs. 95.3%, P=0.07) and complication rates (14.4% vs. 18.6%, P=0.09). Mean haemoglobin drops on postoperative day one were 12 g/L (RIRS) and 17 g/L (mini-PCNL) (P=0.06). Clinically significant bleeding was rare in both groups, with no cases requiring transfusion or embolisation. These findings indicate that RIRS and mini-PCNL are effective and safe options for renal stones when applying the SMASH score.
