
Inflammatory bowel disease (IBD) has seen a rapid rise in Asia, particularly in South Asia and India, over the past two decades, posing significant challenges to healthcare systems in developing nations. A narrative review highlights the shifting epidemiology of adult-onset and pediatric IBD in South Asia, mirroring global trends in disease trajectory. Risk factors for the increasing incidence include urbanization, lifestyle changes, and genetic predisposition. Unique entities like very early-onset IBD and region-specific challenges in diagnosis and management are discussed, emphasizing the need for local solutions. Collaborative research and preventive strategies led by regional experts are critical to understanding the true burden and mitigating its impact on healthcare infrastructure.
