Risk factors for adverse outcomes of endoscopic ultrasound‐guided treatment of pancreatic fluid collections

A recent study analyzed 357 patients undergoing endoscopic ultrasound (EUS)-guided treatment for pancreatic fluid collections (PFCs), focusing on walled-off necrosis (WON) and pseudocysts. The study highlighted specific risk factors leading to adverse events and clinical failures. Adverse events were notably prevalent in cases of WON, with extension to the pelvis and endoscopic necrosectomy as major risk factors. For WON treatment, higher Charlson Comorbidity Index, extension to the pelvis, nonuse of a lumen-apposing metal stent, and percutaneous drainage were associated with clinical failure. Pseudocysts extending to the paracolic gutter and requiring multiple procedures exhibited high risks of clinical failure. Additionally, pseudocysts necessitating the multigateway approach were prone to recurrence. The study emphasized the importance of considering these clinical parameters to predict adverse outcomes during different phases of EUS-guided PFC treatment, suggesting a need for further research to optimize treatment strategies, especially for high-risk patients.
