A study compared the efficacy of ropivacaine and lidocaine in treating temporomandibular joint (TMJ) disorders. Ropivacaine, combined with sodium hyaluronate supra-articular lavage, resulted in faster onset and longer maintenance of anesthesia. The reduction in interleukin-1β levels was more pronounced in the ropivacaine group (16.08 ± 3.10) than in the lidocaine group (18.03 ± 2.84), p < 0.05. At two months, joint clicking rates were significantly higher in the ropivacaine group (75%) compared to the lidocaine group (35%), with similar results at three months (76.69% vs. 40%). Additionally, maximum mouth opening was greater in the ropivacaine group (45.00 ± 2.38) versus the lidocaine group (41.73 ± 4.18), p < 0.05. No significant differences were noted in VAS scores or lateral excursion between groups.
