Safety, tolerability and efficacy of up‐titration of guideline‐directed medical therapies for acute heart failure in elderly patients: a sub‐analysis of the STRONG‐HF randomized clinical trial

The study evaluated the impact of age on the effectiveness and safety of high-intensity care (HIC) strategy in patients admitted for acute heart failure (AHF). The study found that the primary endpoint of death or HF readmission within 180 days was similar in older (>65 years) and younger patients. Older patients initially received slightly lower doses of guideline-directed medical therapy (GDMT) but similar doses later. The efficacy of HIC was numerically higher in younger patients, partially influenced by COVID-19 deaths. HIC improved the quality of life more significantly in younger patients. Overall, HIC was safe and reduced the risk of death or HF readmission across different age groups, but older patients derived smaller benefits in terms of quality of life.
