A study compared the clinical value of SARS-CoV-2 tests performed on saliva samples (SSs) and nasopharyngeal swab samples (NPSs) in predicting COVID-19 disease severity. Samples were collected from hospitalized COVID-19 patients and healthy controls. The study found that SARS-CoV-2 detection rates were slightly higher in NPSs compared to SSs. However, the Ct values (cycle threshold values) of SSs were lower, indicating higher viral load. The first SSs became negative earlier than NPSs. The study also identified that a Ct value ≤30 from SSs was an independent predictor of severe COVID-19. The findings suggest that salivary RT-qPCR testing is suitable for infection control and Ct values can assist in predicting disease severity.
