The study aimed to compare the safety of antipsychotic polypharmacy and monotherapy in patients with schizophrenia across seven dosage categories. The analysis was based on data from the Finnish nationwide inpatient care register of 61,889 patients between 1996 and 2017. Within-individual analysis was used to eliminate selection bias. The study found that antipsychotic polypharmacy was associated with a lower risk of nonpsychiatric and cardiovascular hospitalization at total dosage categories above 1.1 DDDs/day compared to monotherapy. However, there was no significant difference between any polypharmacy use and any monotherapy use. The authors concluded that treatment guidelines should not discourage the use of antipsychotic polypharmacy without existing evidence on safety issues.
