
A prospective observational study evaluated tissue Doppler cardiac imaging (TDI) in extremely preterm infants (<28 weeks gestation) to understand bronchopulmonary dysplasia-associated pulmonary hypertension (BPD-PH). Among 79 infants, 23% had BPD-PH. Those with BPD-PH had a higher incidence of significant patent ductus arteriosus, longer oxygen dependency, and prolonged hospital stays. TDI parameters like left ventricular eccentricity index and acceleration time to right ventricular ejection time ratio showed significant trends. BPD-PH markers were noticeable as early as 33–34 weeks of gestation. Combining TDI with conventional echocardiographic parameters enhances diagnostic accuracy.
