This systematic review and meta-analysis assessed stroke incidence and mortality following head and neck cancer (HNC) treatment. Among 258,850 patients, stroke incidence was 4.1%, higher after chemoradiotherapy (4.9%) than radiation (3.8%). Stroke risk was elevated in older patients and those with hypertension or diabetes. Stroke mortality declined over time but remained highest after CRT (39.3%). Findings support early post-treatment stroke surveillance in HNC survivors.
