A study investigated the prognostic significance of native T1 and extracellular volume fraction (ECV) in AL-cardiac amyloidosis (CA). 38 AL-CA patients underwent 3.0-T cardiac magnetic resonance imaging. Elevated native T1 and ECV correlated with poor prognosis over a median 27-month follow-up, independent of late gadolinium enhancement presence. Multivariate analysis confirmed ECV (HR: 1.37, P = 0.008) and native T1 (HR: 1.01, P = 0.037) as independent mortality predictors. These findings suggest native T1 and ECV's utility in assessing AL-CA prognosis.
