A recent study compared clinical outcomes for patients with MRI-defined T3, clear MRF mid–low rectal cancer; two treatment approaches were evaluated: surgery alone (TME group) and neoadjuvant chemoradiotherapy (nCRT) followed by surgery (nCRT + TME group). The study, comprising 1509 patients, revealed that while nCRT had advantages in sphincter-preserving surgery and tumor downstaging, it resulted in a higher rate of enterostomies. Both approaches demonstrated similar 3-year disease-free survival rates (78.3% TME group, 75.3% nCRT + TME group). The findings suggest comparable long-term oncological outcomes, raising concerns about potential overtreatment with nCRT.
