
A prospective cohort study from the British Association of Dermatologists reveals that among biologic-treated psoriasis patients, those receiving IL-23 inhibitors have the lowest risk of paradoxical eczema. The study, spanning September 2007 to December 2022, included 24,997 exposures, with IL-17 inhibitors showing an incidence rate of 1.22 per 100,000 person-years, compared to 0.94 for TNF inhibitors, 0.80 for IL-12/23 inhibitors, and 0.56 for IL-23 inhibitors. Higher paradoxical eczema risk is associated with increasing age, female sex, and a history of atopic dermatitis or hay fever. Further study is recommended for validation.
