
A study investigated the association between glycemic control and retinal microvascular changes in type 2 diabetes mellitus (T2DM) patients without diabetic retinopathy (DR). Participants were categorized into intensive control (IC) and moderate control (MC) groups based on mean glycosylated hemoglobin (HbA1c) levels. Optical coherence tomography angiography (OCTA) revealed larger foveal avascular zone area and greater choriocapillaris flow deficit (CC-FD) in the MC group compared to IC and controls. Multivariate regression analysis showed CC-FD association with mean HbA1c level. These findings suggest that early choriocapillaris impairment, detected by OCTA, precedes clinically apparent DR and is closely linked to glycemic control, highlighting the importance of monitoring retinal microvascular changes in T2DM patients for early detection and management.
