
A study involving 790 patients with heart failure and preserved ejection fraction (HFpEF) investigated the relationship between left ventricular (LV) myocardial deformation and recurrent hospitalisations for heart failure or cardiovascular death. Using two-dimensional speckle-tracking based global longitudinal strain (GLS), it was found that impaired GLS, indicating LV subclinical dysfunction, correlated with higher baseline N-terminal pro-B-type-natriuretic peptide levels. After a median follow-up of 3.0 years, worse GLS was associated with an increased risk of the primary composite outcome, emphasising the significance of subtle LV systolic impairment in HFpEF pathophysiology.
