Late-onset Alzheimer's disease (LOAD), the prevalent dementia form, disproportionately affects women. Understanding its molecular mechanisms is challenging. The study used human LOAD datasets to analyze cerebral cortex cell populations. Female-specific dysfunctional microglia (FDAMic) were identified and linked to disease severity. FDAMic exhibited traits suggesting active proliferation and amyloid-β binding but defective phagocytosis. They originated from female activated response microglia (ARMic) with APOE4 and compromised estrogen receptor signaling. Insights into ER signaling effects on microglia function were gained. FDAMic association with advanced pathology and cognitive decline suggests a potential therapeutic target for neuroimmune homeostasis restoration.
